Eleven years ago, I was a hopeful intern smiling, energetic, and eager. But over time, those smiles faded, replaced by the daily heartbreak of watching patients suffer in a failing health system.

Iโ€™ll never forget the night a barefoot mother rushed in, clutching her feverish, gasping son. Severe pneumonia. He needed oxygen, antibiotics, tests but the hospital had none. No oxygen. No meds. No lab support. She had no money. I had no choice.

I ran through wards begging. Found one oxygen cylinder in surgery begged again. Dragged it down. Paid for his meds. Paid for his labs. Not because I can afford, but because I had no choice.

He lived. She wept. I cried not from exhaustion, but from despair. Because saving a child shouldnโ€™t demand such desperation.

The next day, no one asked. No one cared. Just another shift. Just another life nearly lost.

And I still ask: Why must we beg to do our jobs?

Weโ€™re not asking for luxury. Just the basics:

Iโ€™m a pediatric and child health specialist, and this year marked the beginning of my fellowship. Like many healthcare workers across the country, Iโ€™ve been deeply involved in the conversation about the Ethiopian health professionals' movementโ€”about our rights, our dignity, and the future of our profession.

Almost everyone around me supported the movement. Some even pushed for a complete strike, refusing to accept halfway measures. I was proudโ€”truly proudโ€”to be among the fellows who stood together with interns and residents, united in striking for what we believe in.

But as days passed, things changed. One by one, people started returning to work after a single call from their departments. Even our most vocal supporters stepped back when senior staff resumed their duties. Our team held out for two weeks. And then, I was the only one left.

Alone, I continued. Not because it was easyโ€”but because it was right.

Itโ€™s scary to stand aloneโ€”and even scarier to become a target. But isnโ€™t the fear of living the same lifeโ€”a life that hasnโ€™t changed for yearsโ€”greater? What has truly changed to justify our return? What questions have been answered? The contempt shown by the government has not only wounded me deeplyโ€”it has made me question whether I can continue in a system that treats its healthcare workers with such disregard.

Why do we fear standing alone? Are we not just worth & powerful enough as a person? Why do we wait for others to act for what we believe is right? Is it not true that we each ultimately benefit on an individual level when systemic change happens? Even one voice, one stand, one refusal matters. Others must be allowedโ€”and expectedโ€”to carry their share of the burden. It is not just ours to carry.

I feel the weight of every question thatโ€™s been raised during this strike. And I cannot imagine walking back into work with disgrace, knowing none of those questions have been answered.

Letโ€™s be clear: the interns and residents are not striking for themselves alone. They are raising the very concerns we have all carried in silence for years. Their voices speak for a broken system that has for too long failed those who hold it together. Many are frightenedโ€”not just for their personal survival, but for their futures, as they watch seniors scraping by, preparing for foreign exams, or migrating in hopes of escape.

When our residents tell us, โ€œI canโ€™t afford to eat,โ€ how can we not respond with solidarity?
And yet, rather than support them, some seniors are being used to pressure themโ€”to coerce, guilt, and shame them back into submission. I cannot, in good conscience, be part of that betrayal. As a senior and fellow, I know that their fight is also our fight. Their victory will be ours too. If anything, we should be the ones leading this movementโ€”not standing in the way of it.

I will not be complicit in normalizing suffering, nor will I help conceal the cracks in a system that is clearly collapsing.
Please dont just be a follower hiding behind the crowd. Stand up for your right even if you are alone.

This is a rare moment. A real opportunity for change. If we waste it out of fear or convenience, we forfeit the right to ever complain again.
So yes, I will stand alone, if I must. And I hope others will find the strength to stand alone tooโ€”not just from duty, but from a deep belief that we deserve better. All of us.

It is with pride and unwavering resolve that I say: I will not return to clinical work before the interns and residents do. I will not help cover up a crisis or pretend things are normal when they are not. We either stand together nowโ€”or accept whatever comes in silence later.

They let you close the fascia and skin, then drive off in their luxury carsโ€”leaving you stranded in the middle of the night, searching for a taxi, hungry and exhausted. The only relief is making it home safely, only to become sleepless thinking about the morning report youโ€™ll present to the very same senior who left you behind.

What can you expect from seniors who expect you to cover their private outpatient clinics without pay, placing you in a moral and professional dilemmaโ€”do you speak up and risk academic consequences, or stay silent and continue to be exploited?

What can you expect from seniors who refer patients from their private hospitalsโ€”patients theyโ€™ve already financially drainedโ€”leaving you to manage the complications alone?

Even when some seniors appear to support your efforts or movements, it often isn't out of genuine concern, but because the workload they once offloaded onto residents and interns has finally returned to them.

With all due respect to our seniors and consultants, we must acknowledge the truth: they share responsibility for many of the systemic failures within our healthcare system.

We are aware that there are individuals actively working to sabotage our unity and create confusion. These people, falsely claiming to represent the EHPM Board, are spreading misinformation โ€” including false claims that the strike has ended. Do not listen to them. These actors have been given a mission to divide and disintegrate our movement.

Dear colleagues, we urge you to remain vigilant and follow only the directives issued by the EHPM Central Board, and only through its official channels.

Furthermore, we strongly call on chief residents who are pressuring and intimidating their fellow residents to return to work โ€” cease these shameful and destructive actions immediately. You are undermining a national cause and betraying the collective sacrifice of thousands of your colleagues.

We also emphasize: do not participate in any institution-based meetings. Our demands are national in nature and must be addressed at the national level.
