بشكل عام كل خلايا الجسم إلها Autoantigen
وجهاز المناعه عند الانسان الطبيعي بيكون عارف ال Autoantigen تعت خلايانا وما بيهاجمهاا
انما لو صار خلل او mutation بمكان معين لخلايا المناعه ، هتفقد خلايا المناعه قدرتها على التعرف على ال Autoantigen الخاصه بالجسم
فبتبتدي خلايا المناعه يتهيأ لها ان ال autoantigen , جسم غريب وبتروح تهاجمه وتدمره ...

ف في ال AIH بيكون عند المريض mutation في ال HLA2
الموجوده على خلايا المناعه
فبيروح جهاز المناعه يهاجم ال autoantigen موجوده بال liver
🌸زي مين ال Autoantigen الموجوده بال liver اللي هيهاجمها جهاز المناعه : ⬇️⬇️
CYP2D6 ، PANCA ، ASGPR ، FTCD ، α-SMA ،SLA

بالتالي بتتكسر خلايا الكبد والخلايا لما تتكسر هتتحوط ب Inflammatory cell , وهيعلى ال AST/ALT
وهيصير ما يسمى Interface Hepatitis ..

Def / AIH : Loss of Tolerance against liver Antigen that is Triggered by Environmental factor in Individuals with Genetic Susceptibility

شو هو ال pathogenesis للAIH ..

1️⃣ المريض بيكون عنده Genetic mutaion

الجين اللي بيصيرله فيه mutation موجود بال HLA2 الموجوده بال Immune cell
HLA-Dr4 , HLA-DR4 ,..

2️⃣ بيتعرض المريض ل Environmental factor
🔸️زي ال viruses
HAV , HBV ,HCV , CMV ,Other
جهاز المناعه لما يتعرف ع الفيروسات ويتخلص منها ، بيتهيأ له انه ال hepatocyte antigen , هو نفسه ال virus antigen فبيروح يدمرهاا

🔸️حالات ال AIH بيكون عندهم زياده بال Permiability لل Intestinal microbiota, فبالتالي تزيد ال Translocation للبكترياا ناحية ال portal vein

🔸️بعض الأدويه لما يصيرلها metabolisim بالكبد ،جهاز المناعه بيفكرها antigen ,وبيروح يهاجم خلايا الكبد
زي ال Nitrofurantoin ,وال statin , وال Infliximab

3️⃣ بسبب ال Gene mutation وال Environmental factor ,هينشط ال Immune system ويبدأ يطلع Cytokines و Antibody لتهاجم ال Hepatocyte بدون توقف ..

amplification of liver damage by several mechanisms, including

🔸️ CD8 lymphocytes secrete interferon gamma and stimulate cytotoxicity

🔸️ Th17 cells secrete interleukin 17 which is hepatotoxic

🔸️Increased number of gamma delta T cells, especially during active disease phases, and their expression of granzyme B (an effector-cell molecule) directly correlates with biochemical indices of liver damage, such as elevated alanine aminotransferase (ALT) and bilirubin

🔸️Induced expression of HLA class II molecules on hepatocytes allows them to act as antigen-presenting cells and contribute to perpetuation of liver damage

» Management Hypernatremia in ICU:

•Remember that most common reason for Hypernatremia in Critical care is Iatrogenic so search for hyperosmolar fluids include NaHCO3✓
•other potential cause for Hypernatremia after cerebro-Vascular insult is Diabetes insipidus.
( mostly will be Cranial Diabetes insipidus but also need to rule out possible NDI).

•you have to measure Serum osmolality/urine osmolality, UOP and Urine Na level.

» Lets give an example for Correction of Hypernatremia ( Na level 160meq/L):

1- Calculate Water deficit:
TBW × ( Current Na/ 140)-1
Total Water deficit:
0.6 × 70 × ( 160 /140)-1 = 6 Litres

2-The Water needed to be given or Volume of fluid needed to Change Na level no more than 0.5 meq/L/h and no more than 6-8 meq/L per 24 hours.

So, the volume of fluid needed to correct Na level to 152 meq/L in 24 hours =
TBW × ( Current Na- Desired Na / 140 )
42 × ( 160- 152 /140)= 2.4 L ~ 2400 ml /24h.
» Add to that insensible losses estimated ~500-800 ml/day
so lets agree it is around 3000ml per day.
so the rate per hour ~ 125 ml/h ( Can give Tap Water or D5% with sterile water).

ال Immune System لما يبتدي يهاجم ال Hepatocyte ,هيبدأ يطلع عنا Antibody اشهرهم :
🟣 ANA : Anti Nuclear Antibody

🟢 ASMA : Anti smooth Muscle Antibody

🔵 LKM : Anti liver Kideny Microsomal Antibody

🟡 LC1 : Anti liver Cystol Type 1

🟠 Anti SLA : Anti soluble Liver Antigen

🟤 anti LP : Anti liver Pancreas Antigen

🔴Anti-neutrophil cytoplasmic antibodies (ANCA)

⚫️Anti-chromatin, anti dsDNA, anti-CCP, anti-ASGPR.
كل التسميات لل Antibody ،اجت على اساس ال enzyme او الجزء اللي بالخلية اللي بيتهاجم وبيمسك فيه ال Antibody ..

هل هادي ال Antibody تعتبر Specific في مريض ال AIH???
لا ، لا تعتبر specific
وهتكون Positive في امراض كتيره غير ال AIH
على سبيل المثال ال ANA ,ييكون Positive في الحالات التاليه :
SLE ,PBC ,PSC , Alcoholic Liver Disease

وال ASMA بيكون + في امراض ال Rheumatic والانفكشوس
لكن ال Antibody هتساعدني في تشخيص الAIH ..
ف 80% من مرضى ال AIH هيكون ال ANA عندهم Positive
و 60% من حالات ال AIH بيكون عندهم ASMA ايجابي
و 3% من حالات ال AIH بيكون ال LKM ايجابي

ف لو مريض شاكيين انه AIH, بنطلبله ANA,ASMA,LKM
لو كانو اتنين منهم + هاد بيعزز التشخيص
اما لو كان بس واحد منهم + ,ف احتمال يكون AIH او لأ ..
