I am writing to provide a discharge summary for Mr Lionel Ramamurthy, a 63-year-old resident of the Community Retirement Home, who was admitted to Newtown Public Hospital on 4 February 2019 with a diagnosis of pneumonia. He is scheduled for discharge tomorrow and will return to your care.
Upon admission on 4 February 2019, Mr Ramamurthy presented with a three-day history of fever, rigors, shortness of breath, wheezing, and a persistent dry cough. He also reported generalized body aches, abdominal discomfort, and fatigue. At the time, he was functionally dependent, mobilising with a pick-up frame, and required assistance with activities of daily living, including showering and dressing. Additionally, he appeared frail with a poor appetite and recent unintentional weight loss.
During his hospital stay, Mr Ramamurthy was treated with intravenous antibiotics and supportive care, resulting in the resolution of fever and stabilization of respiratory function. Chest physiotherapy was initiated to aid airway clearance, and he responded positively, with a significant reduction in coughing and wheezing. A tailored nutrition plan was implemented following a dietetic review, leading to a 1.5kg weight gain. With consistent physiotherapy and nursing support, he regained independence in personal care and gradually progressed to mobilising without assistance. He is currently afebrile, hemodynamically stable, and no longer requires supplemental oxygen.
Upon discharge, Mr Ramamurthy will require continued support to maintain the progress achieved during his hospitalization. He should be encouraged to remain well hydrated, consume a nutritious, protein-rich diet, and sit upright when possible to promote effective lung drainage. Breathing and coughing exercises are to be continued as instructed by the physiotherapy team. Paracetamol may be administered as needed for residual chest or abdominal discomfort. Although he has regained independence in personal care and mobility, light supervision during ambulation is advised. Given his recent bereavement, ongoing emotional support and social engagement may also be beneficial.
