The Verghese Odyssey: An inspiring story
Abraham Verghese, born in Addis Ababa, Ethiopia from expatriate Indian family in 1955, is a highly respected American physician, world class author, humanitarian, professor of medicine and vice chair at the renowned Stanford University.
He began his medical school in Ethiopia but moved to USA due to political unrest when the emperor was deposed by Derg regime. He later completed his medical training in madras medical college in India in 1979 and came back to USA as a foreign medical graduate and completed his residency in internal medicine in Tennessee and infectious disease fellowship at Boston university school of medicine.
He is an eloquence author of two memoirs and two novels which are New York Times best seller. His first book , my own country , narrates his memoir as a young physician during the HIV pandemic.
His second book, the tennis partner is an autobiographical memoir of a troubled drug addict physician that he met as a mentor amidst Verghese unraveling first marriage.
Cutting for stone, his third book is published 9 times and translated in to 16 different languages. It eloquently narrates the saga of twin brothers orphaned by their mother death at birth. The story deeply explores human sides of medical practice plotting in Ethiopia.
His 4th novel which is stated as the most anticipated book by Washington post has been released last month with the title ‘ the covenant of water‘. It depicts the shimmering evocation of lost India and the passage of time itself.
Dr Verghese has coined the ‘l patient ‘to demonstrate the scared ritual of medical practice, physical examination , which is at the verge of extinction as current medical practice mainly focus on treating the patient in the computer (the i patient ) than the real patient in bed. He is very well known for his motto, ‘ imagining the patient experience’.
He is a man of empathy, compassion and integrity. He has received honorary doctorate degree from 5 prestigious universities. He received Heinz award in 2014, national humanities medal presented by President Barack Obama in 2015 and recently won the 2023 writer in world prize for embodying a rare combination of literary talent and moral imaginations helping us to better understand the world and our place in it.

ሰላም ጤና ይስጥልኝ!
ዛሬ ለካንሰር ህክምና አንዱ ምሶሶ ስለሆነው የኬሞቴራፒ ህክምና የተወሰኑ ነገሮችን እንማማራለን።
1) ኬሞቴራፒ ምንድነው?
ኬሞቴራፒ ማለት የኬሚካል ህክምና ሲሆን ህክምናውም በፍጥነት እያደጉ ያሉን ህዋሶችን በጠንካራ ኬሚካሎች የምንገልበት መንገድ ነው። ኬሞቴራፒን በብዛት የምንጠቀመው ካንሰርን ለመግደል ነው። ይህም የሆነበት ምክንያት የካንሰር ህዋሶች በፍጥነት የመባዛትና የማደግ ባህሪ ስላላቸው ነው።
በካንሰር ህክምና ውስጥ ብዙ አይነት የኬሞቴራፒ መድኃኒቶች ሲኖሩን እነዚህን መድኃኒቶች ለየብቻ ወይም ተደባልቀው ሊሰጡ ይችላሉ።
ኬሞቴራፒ ወይም የኬሚካል ህክምና ብዙ የካንሰር አይነቶችን የመግደል ብቃት ሲኖረው እራሱን የቻሉም የጎንዮሽ ጉዳቶች አሉት።
እነዚህም የጎንዮሽ ጉዳቶች ቀላል እና ሊታከሙ የሚችሉ ሲሆኑ አንዳንዶቹ ጉዳቶች ግን ከባድ እና ለህክምናም አዳጋች ናቸው።
2) የኬሚካል ህክምናው(ኬሞቴራፒ) ለምን ይሰጣል?
ኬሞቴራፒ በዋነኝነት የሚሰጠው ካንሰር ያጠቃቸውን የካንሰር ህዋሶች ለመግደል ነው። ይህም ህክምና የተለያዩ መልኮች ይኖሩታል።
እነሱም፦
I) ሙሉ በሙሉ ካንሰሩን ለማጥፋት የሚሰጥ የኬሚካል ህክምና(Curative or Radical Chemotherapy)
- ኬሞቴራፒን በብቸኝነት ያለ ሌላ ህክምና ዕገዛ ካንሰሩን ለማጥፋት የምንጠቀምበት አግባብ ነው።
II) ከሌላ ህክምና በኋላ የሚሰጥ አጋዥ የኬሚካል ህክምና(Adjuvant Chemotherapy) ፦
ይህም ማለት ታካሚው የቀዶ ጥገና ከተደረገለት ወይም ካንሰሩን የማዳከም ህክምና ከተደረገ በኋላ የሚሰጥ ህክምናን ያካትታል። ይህም ህክምና ካንሰሩን ሙሉ ለሙሉ ለማጥፋት ይረዳል።
III) ለዋንኛው ህክምና ለማዘጋጀት የሚሰጥ የኬሚካል ህክምና(Neoadjuvant Chemotherapy) :-
ኬሞቴራፒው የካንሰሩን መጠን ለመቀነስ እና በኋላ ላይም ዋንኞቹን የህክምና አይነቶች(ለምሳሌ፦ የጨረር ህክምና ወይም የቀዶ ጥገና) በቀላሉ እንዲደረጉ ማመቻቸትን ያጠቃልላል።
IV) የካንሰር በሽታ ህመምን እና ምልክቶቹን ለመቀነስ የሚደረግ ህክምና፦
ኬሞቴራፒ የበሽታውን ስቃይና ህመም እንዲቀንስ ተብሎ የሚሰጥበት አግባብ ነው። ይህም ህክምና ፓሊዬቲቭ ኬሞቴራፒ ወይም ስቃይን የመቀነስ ህክምና ተብሎ ይታወቃል።
3) የኬሚካል ህክምና ከካንሰር በሽታ ውጪ ሊሰጥ ይችላል?
የህክምናው ጠበብቶች የዚህን የኬሚካል ህክመና ከካንሰር በሽታ በተጨማሪ ለሌሎች በሽታዎች ህክምናም ይጠቀሙባቸዋል። ከእነኚህም በሽታዎች መካከል የመቅኔ እና በሽታን የመከላከል አቅምን ለሚጎዱ ይገኙበታል።( Bone Marrow Disease and Immune System Disorders)
4) የኬሞቴራፒ የጎንዮሽ ጉዳቶች ምን ይመስላሉ?
የኬሚካል ህክምናው የጎንዮሽ ጉዳቶች እንደየ ህክምናው አይነት ቢለያዩም ብዙዎቹ ግን የመመሳሰል ባህሪ አላቸው። እያንዳንዱ ኬሚካል እራሱን የቻለ የጎንዮሽ ጉዳት ሲያመጣ፤ ውህድ ሆነው ተደባልቀው ሲሰጡ ደግሞ ጉዳታቸው በዛኑ ያክል ይጨምራል።
የጎንዮሽ ጉዳቶቹም ይህን ይመስላሉ፦
ሀ) ማቅለሽለሽ
ለ) ማስመለስ
ሐ) ተቅማጥ ወይም የሰገራ መለስለስ
መ) የፀጉር መመለጥ
ሠ) የምግብ ፍላጎት መቀነስ
ረ) ድካም
ሰ) ትኩሳት
ሸ) የአፍና የከንፈር ቁስለት
ቀ) ህመም
በ) የሆድ ድርቀት
ተ) የቆዳ ቁስለት
ቸ) የመድማት ችግር እና ወ.ዘ.ተ
እነዚህ የጎንዮሽ ጉዳቶች ልንከላከላቸው የምንችላቸው እና ሊታከሙም የሚችሉ ናቸው። ብዙዎቹ የጎንዮሽ ጉዳቶችም ከህክምናው በኋላ የሚጠፉ ናቸው።
5) ከኬሚካል ህክምና በኋላ ላይጠፉ የሚችሉ የጎንዮሽ ጉዳቶችስ አሉ?!
የኬሞቴራፒን ህክምና ተከትሎ የሚመጡ የጎንዮሽ ጉዳቶች ከወራቶች እንዲሁም ከዓመታት በኋላ ሊታዩ ይችላሉ። እነዚህም ጉዳቶች የዘገዩ የህክምናው ጉዳቶች( Late Side Effects) በመባል ይታወቃሉ።
ከእነኚህም መካከል፦
i) የሳንባ ህዋሶች ጉዳት
ii) የልብ ችግር
iii) መካንነት
iv) የኩላሊት ችግር
v) የነርቭ ጉዳት እና
vi) ህክምናውን ተከትሎ ከረጅም ዓመታት በኋላ የሚከሰት የጎንዮሽ ካንሰር ሊሆኑ ይችላል።
ከላይ የተጠቀሱት የጎንዮሽ ጉዳቶች ለታካሚው በተሰጠው የኬሚካል አይነት፣ የኬሚካል መጠን እና ውህድ ላይ ይመረኮዛሉ። የእነዚህ ችግሮች ምልክት የታየበት ግለሰብ ሐኪሙን እንዲያማክር ይበረታታል!
6) ለኬሞቴራፒ ህክምና ምን አይነት ቅድመ ዝግጅት መደረግ አለበት?!
ቅድመ ዝግጅቱ እንደየ ኬሚካል ህክምና ባህሪ እና የልኬት መጠን ሲለያዩ ህክምናውም የሚሰጥበት መንገድ ላይ ይመረኮዛል።
መደረግ ያለበት ቅድመ ዝግጅት፦
ሀ) ጠቅላላ የሰውነት ምርመራ እና ተጨማሪ በሽታዎች አለመኖራቸው ሊታወቅ ይገባል።
ለ) የታካሚውን ሙሉ ጤነኝነት ለማረጋገጥ አስፈላጊ የላብራቶሪ ምርመራዎች መደረግ ይኖርባቸዋል።
ሐ) የጥርስ ህክምና ምርመራ ሊደረግ ይገባል።
ይህም የጥርስ ኢንፌክሽን ካለ በቀላሉ ለማከም ይጠቅማል።
መ) ህክምናው ሳይጀምር በፊት ስለጎንዮሽ ጉዳቶቹ በቂ ምክክር ከካንሰር ስፔሻሊስት ሐኪሞቹ ጋር ሊደረግ ይገባል።
ሠ) የኬሚካል ህክምናው ከመደረጉ በፊት ቤት ውስጥ እና መስሪያ ቤት ላለ ስራ አስፈላጊውን የትብብር ዕርዳታ መስጠት የሚችል አጋዥ(ተለዋጭ) ሰው ማዘጋጀት ያስፈልጋል።
ረ) የመጀመሪያው የኬሚካል ህክምና ከመደረጉ በፊት ከጤና ባለሞያዎች ጋር ስለ አመጋገብ እና የኬሞቴራፒ ህክምና በሚደረግበት ወቅት ስለሚደረግ ቅድመ ጥንቃቄ መመካከር ያስፈልጋል።
7) በካንሰር ህክምና ላይ ከሚሰጡት ብዙ የኬሚካል አይነቶች ውስጥ የኬሞቴራፒ አይነቱን ሐኪሙ እንዴት ይመርጣል?
የካንሰር ሐኪሙ የኬሚካል ህክምናው ከመጀመሩ በፊት የኬሞቴራፒ አይነቶቹን መርጦ ይወስናል። ይህም ምርጫ፦ በካንሰሩ አይነት፣ በካንሰሩ ደረጃ ላይ፣ በታካሚው ሙሉ ጤንነት ላይ እና የድሮ ህክምና ላይ ተመርኩዞ የኬሚካል አይነቶቹን ይመርጣል።
8) ኬሞቴራፒ ወይም የኬሚካል ህክሜናው የሚሰጥባቸው መንገዶች ምን ይመስላሉ?
i) በደም ስር የሚሰጥ የኬሚካል ህክምና
ii) በአፍ የሚዋጥ የኬሚካል መድኃኒት
iii) በፍጥነት በደም ስር የሚሰጥ የኬሚካል ህክምና
iv) ቆዳ ላይ የሚቀባ የኬሞቴራፒ ክሬም
v) በሰውነት አካል ውስጥ በቀጥታ ገብቶ የሚሰጥ የኬሚካል ህክምና
vi) በቀጥታ ካንሰሩ ላይ የሚወጋ የኬሚካል መድኃኒት ሊሆኑ ይችላሉ።
9) የኬሚካል ህክምናው በጊዜ ሂደት አሰጣጡ ምን ይመስላል?
የካንሰር ህክምና ጠበብቱ ኬሞቴራፒው ለስንት ጊዜ ያክል መሰጠት እንደሚችል እና በየስንት ግዜውም መሰጠት እንዳለበት ይወስናል። ይህ ውሳኔ በካንሰር በሽታው ባህሪ እና አይነት ላይ ተመርኩዞ ይደረጋል። በብዛት በሀገራችን የሚሰጠው የኬሞቴራፒ ህክምና ዑደት በየ 21 ቀኑ ሲሆን ህክምናውም ቢያንስ ለስድስት ዙር ያክል ይደረጋል።
10) ኬሞቴራፒ የት ይሰጣል?
የኬሚካል ህክምና በማንኛውም የካንሰር ማዕከል ሊሰጥ ይችላል። ማዕከሉም የግዴታ በካንሰር ህክምና ላይ በሰለጠኑ ስፔሻሊስት ሐኪሞች እና የMSC Oncology ነርሶች ሊገለገል ይገባል። ኬሞቴራፒ ካለው የጎንዮሽ ጉዳት አንፃር ኬሞቴራፒውን ማዘጋጃ Chemo Hood or Biosafety Cabinet በማዕከሉ ሊገኝ ይገባል። በማዕከሉም የሚገኙት የጤና ባለሞያዎች ታካሚውን ከመርዳት ባሻገር እራሳቸውንም ከኬሞቴራፒ የጎንዮሽ ጉዳቶች መጠበቅ አለባቸው!

ዛሬ ለካንሰር ህክምና አንዱ ምሶሶ ስለሆነው የኬሞቴራፒ ህክምና የተወሰኑ ነገሮችን እንማማራለን።
1) ኬሞቴራፒ ምንድነው?

ለ) የታካሚውን ሙሉ ጤነኝነት ለማረጋገጥ አስፈላጊ የላብራቶሪ ምርመራዎች መደረግ ይኖርባቸዋል።
ሐ) የጥርስ ህክምና ምርመራ ሊደረግ ይገባል።
ይህም የጥርስ ኢንፌክሽን ካለ በቀላሉ ለማከም ይጠቅማል።
መ) ህክምናው ሳይጀምር በፊት ስለጎንዮሽ ጉዳቶቹ በቂ ምክክር ከካንሰር ስፔሻሊስት ሐኪሞቹ ጋር ሊደረግ ይገባል።
ሠ) የኬሚካል ህክምናው ከመደረጉ በፊት ቤት ውስጥ እና መስሪያ ቤት ላለ ስራ አስፈላጊውን የትብብር ዕርዳታ መስጠት የሚችል አጋዥ(ተለዋጭ) ሰው ማዘጋጀት ያስፈልጋል።
ረ) የመጀመሪያው የኬሚካል ህክምና ከመደረጉ በፊት ከጤና ባለሞያዎች ጋር ስለ አመጋገብ እና የኬሞቴራፒ ህክምና በሚደረግበት ወቅት ስለሚደረግ ቅድመ ጥንቃቄ መመካከር ያስፈልጋል።
7) በካንሰር ህክምና ላይ ከሚሰጡት ብዙ የኬሚካል አይነቶች ውስጥ የኬሞቴራፒ አይነቱን ሐኪሙ እንዴት ይመርጣል?
የካንሰር ሐኪሙ የኬሚካል ህክምናው ከመጀመሩ በፊት የኬሞቴራፒ አይነቶቹን መርጦ ይወስናል። ይህም ምርጫ፦ በካንሰሩ አይነት፣ በካንሰሩ ደረጃ ላይ፣ በታካሚው ሙሉ ጤንነት ላይ እና የድሮ ህክምና ላይ ተመርኩዞ የኬሚካል አይነቶቹን ይመርጣል።
8) ኬሞቴራፒ ወይም የኬሚካል ህክሜናው የሚሰጥባቸው መንገዶች ምን ይመስላሉ?
i) በደም ስር የሚሰጥ የኬሚካል ህክምና
ii) በአፍ የሚዋጥ የኬሚካል መድኃኒት
iii) በፍጥነት በደም ስር የሚሰጥ የኬሚካል ህክምና
iv) ቆዳ ላይ የሚቀባ የኬሞቴራፒ ክሬም
v) በሰውነት አካል ውስጥ በቀጥታ ገብቶ የሚሰጥ የኬሚካል ህክምና
vi) በቀጥታ ካንሰሩ ላይ የሚወጋ የኬሚካል መድኃኒት ሊሆኑ ይችላሉ።
9) የኬሚካል ህክምናው በጊዜ ሂደት አሰጣጡ ምን ይመስላል?
የካንሰር ህክምና ጠበብቱ ኬሞቴራፒው ለስንት ጊዜ ያክል መሰጠት እንደሚችል እና በየስንት ግዜውም መሰጠት እንዳለበት ይወስናል። ይህ ውሳኔ በካንሰር በሽታው ባህሪ እና አይነት ላይ ተመርኩዞ ይደረጋል። በብዛት በሀገራችን የሚሰጠው የኬሞቴራፒ ህክምና ዑደት በየ 21 ቀኑ ሲሆን ህክምናውም ቢያንስ ለስድስት ዙር ያክል ይደረጋል።
10) ኬሞቴራፒ የት ይሰጣል?
የኬሚካል ህክምና በማንኛውም የካንሰር ማዕከል ሊሰጥ ይችላል። ማዕከሉም የግዴታ በካንሰር ህክምና ላይ በሰለጠኑ ስፔሻሊስት ሐኪሞች እና የMSC Oncology ነርሶች ሊገለገል ይገባል። ኬሞቴራፒ ካለው የጎንዮሽ ጉዳት አንፃር ኬሞቴራፒውን ማዘጋጃ Chemo Hood or Biosafety Cabinet በማዕከሉ ሊገኝ ይገባል። በማዕከሉም የሚገኙት የጤና ባለሞያዎች ታካሚውን ከመርዳት ባሻገር እራሳቸውንም ከኬሞቴራፒ የጎንዮሽ ጉዳቶች መጠበቅ አለባቸው!
