تنقسم حالات hypertensive crisis التي تحضر الى مركز الحوادث والطوارئ
إلى مجموعتين
# hypertensive urgency
# hypertensive emergency

الثلاث الفروقات الأساسية بينهما كالتالي
■hypertensive urgency :
1.without end organ damage.
2. oral treatment.
3. out patient follow up

■hypertensive emergency :
1. with end organ damage.
2. parentral treatment.
3. needs admission

#end organ damage means (at least one of the following ):
. cardiac :
1.acute coronary syndrome.
2. acute pulmonary odema
3. aortic dissection.

neurological :
1. stroke ( ischemic, heammoragic ).
2. TIA
3. hypertensive encephalopathy

retinal :
papillodema ( malignant hypertension ).

renal :
rapid rise of urea, and creatinin.

hematology :
microangiopathic hemolytic anemia.

لا يوجد اتفاق حول كم يكون الضغط في الساعات الأولى عند مرضى hypertensive emergency
all guidelines are conflicting
لكنهم مجمعين على
1. don't rapidly normalize blood pressure, because of it's harmful effects.

2.reduce 25 % from systolic in the first hour
كما قال ذلك أيضا ميدسكاب
يعني لو ضغط المريض systolic كان 200
ينقص إلى 150 خلال الساعة الأولى.

العلاجات المستعملة كثيرة مثل :
soduim nitropruside .
labetalol .
nicardipin
enaloprilat
fenoldopam
nitroglycerin
hydralazine.

العلاجات الغير مذكور جرعاتها في المنشور يرجع لها في المراجع
hypertensive emergency
المتوفر للاسف في بلدنا فقط ثلاثة أنواع
■.labetolol :
ampoul 20 mg
الجرعة القصوى 300 mg
I.v injection direct
20 , 40 , 80 ,80 ,80 mg
يفصل بين كل جرعة وآخرى 10 minutes

مع متابعة الضغط
or given infusion ( 1 -2 mg /minute )
200 mg labetalol in 200 ml saline ( 1 mg =1 ml )
20 drops / minute.

■nitroglycerin infusion pump
50 mg diluted in 50 ml dextrose 1ml / hr start maximum 12 ml / hr
( 10 mcg /hr - 200 mcg /hr )
كل عشر دقائق ضيف واحد ملي بالساعة
مع متابعة الضغط.

■hydralazine ampoul 20 mg
5 mg injection i.v direct repeated every 15 minute maximum 20 mg
ميدسكاب قال 30 mg

#FUO was originally defined by Petersdorf and Beeson in 1961 as an illness of >3 weeks' duration with fever of >=38.30C (1010F) on two occasions and an uncertain diagnosis despite 1 week of inpatient evaluation.

👉The definition of #FUO has been further modified by the exclusion of immunocompromised patients,whose workup requires an entirely different diagnostic and therapeutic approach.

#FUO is now defined as:
🦠 Fever >38.30C (1010F) on at least two occasions
🦠 Illness duration of 3 weeks
🦠No known
immunocompromised state
🦠Diagnosis that remains uncertain after a thorough history-taking, physical examination,

👉the following obligatory investigations:
-(ESR
-CRP level;
-CBC
-electrolytes,
-creatinine,
-total protein, alkaline phosphatase, ALT, AST, LDH,
creatine kinase, ferritin,
-ANA,RF
-protein electrophoresis
-GUE
-blood cultures.

قبل استقبال حالة septic shock
في مجموعة مصطلحات ضروري الإلمام بها

■ sepsis or septicemia
تحتاج توفر حاجتين

1. presumed or confirmed infection
عادة يكون bacterial.
2. أثنين من إلا ربع الحاجات التي يشملها
●SIRS ( systemic inflammatory response syndrome )
a. temperature >38 or <36
b. heart rate > 90
c. respiratory rate > 20
d. WBC >12000 or <4000 or band forms >10%
الآن دة استبدلوة في 2016 ب criteria جديدة اسمها
●q SOFA
( quick sequential organ failure assessment )
ولازم كلها تكون موجودة حتى تقول sepsis
a. altered mental state
b. respiratory rate equal or > 22
c. systolic pressure equal or <100

اذا رافق sepsis اي خلل في
kidney function, liver function, or platelet
with normal blood pressure
يسمى
■severe sepsis

اذا رافق severe sepsis
hypotension
MAP ( mean arterial pressure )< 65 mmHg or systolic pressure <100 mmHg
لم يتحسن بعد ضخ adequate fuilds
واستجاب إلى vasopresser
هنا يسمى
■ septic shock
اذا رافق ذالك failure in more than one organ
يسمى MODS
multiple organ failure syndrome

●septic shock treatment:

■ infuse 30 ml /kg saline or ringer lactat
fast over one hour repeated if needed
1 liter to 2 liter over 1 hours
خلال large bore cannula
خضرا وردي او بيضاء اللون .
اذا المريض استلم تقريبا ٤ التار خلال بضعة ساعات
تستخدم noradrenaline ■
it is the best vasopresser in septic shock
noradrenaline ampoul ( 8 mg ) in 50 ml fluid by syringe pump 1ml /hr - 20 ml / hr
1- 20 mcg / minute

■ antibiotics should be given in the first hour if you suspect septic shock in ER department
لازم يغطي gram negative and positive bacteria ويغطي كمان anaerobic bacteria
if you suspect intraabdominal or skin infection
immunocompetent patient can be given one antibiotic
immunocompromised better to be given two antibiotic
الخيارات المتاحة في septic shock تعتمد على possible bacteria
1. meropenem 1gram TDS injection or
2.pipracilline tazobactam 4.5 gram infusion TDS or 6 hourly or
3. vancomycin 1gram infusion 12 hourly if suspect MRSA in patient with central line
ارسل عينة blood culture قبل إعطاء anti biotic
لكن لا تتاخر في إعطائة إلى أكثر من ساعة
■ steroid
hydrocortisone 100 mg injection 8 hours for 3 days
تذكر بعض المراجع استخدامها لمايحصل من
adrenal insufficiency associated with septic shock
لكن لا يوجد إجماع في ذالك

👉❗️Based on the level of psychomotor activity #delirium can be described as :
🔻hyperactive,
🔻hypoactive, or
🔻mixed.

🔎Hyperactive delirium
-is observed in patients in a state of alcohol withdrawal or intoxication with phencyclidine (PCP),amphetamine, and lysergic acid diethylamide (LSD).
-These patients often exhibit agitation, restlessness, hallucinations, or delusions. 

🔎Hypoactive delirium
-is observed in patients in states of hepatic encephalopathy and hypercapnia and may be more common in older adults. 
-Hypoactive delirium presents with lethargy, drowsiness, apathy, decreased responsiveness, or slowed motor skills. 

🔎mixed delirium
-individuals display either relatively normal levels of psychomotor activity or rapidly fluctuating levels of activity.
